Why will this not go away?

I had had issues with my stomach for years now. It happens when it wants but it is so painful. When i eat or not eat. I get these sudden bowel movements, and they are painful. I have had a ultrasound done on my gall bladder and it is fine. I cant stand it any more, what can i do?
